Please don’t forget to have an up to date consent form. It is important that we have your up to date contact details in case of an emergency. We can electronically send your consent form by emailing us and we can then send you one direct from Docusign. Easy peasy 🙂
Or you can come in and pick up a consent form and complete it that way.
Please remember that it must be a parent or legal guardian that completes the form and we do randomly ask for proof of age of your child.
